SAFETY TRAINING  ISSUES THAT MUST BE ADDRESSED VIA DOCUMENTED TRAINING

 

1. Read and know the operational procedures/protocols pertinent to this unit.

 

2.  Review all safety procedures and practices particular to this unit, including emergency shut-off procedures.  Know the location of the main power emergency cut-off switch and when to use it. Discuss the potential physical, electrical and chemical hazards and control of such hazards. Know that the supervisor must be immediately informed of any unusual occurrences or malfunctioning of the unit.

 

3.  Review Title 17, CCR, section 30336 (a) (1) “Radiography Employing Radiation Machines” (attached).

 

4.  Review prohibited practices such as overriding interlocks and repairing the unit without written permission from the Campus Radiation Safety Officer, or resuming use of the unit after repair without permission from Campus Radiation Safety.

 

5.  Review dosimetry requirements associated with the use of this unit, if any. 

 

6.  Review the appropriate wording and location of any required signs and labels.  Be aware that extra signs and labels are available in the Radiation Safety Office.

 

7.  Discuss the responsibilities of the Supervisor (see below).

 

8.  Discuss the responsibilities of Personnel/Students (see below) who will operate or be in the presence of an operating unit.

 

9.  Discuss emergency procedures (call 911) and available sources of information [Primary Investigator; Project Manager; CSULB Radiation Safety Office, x55623; Radiological Health Branch (800) 852-7550.

 

 


RESPONSIBILITIES OF THE PRIMARY INVESTIGATOR

 

1.  Must ensure that all users receive adequate and appropriate safety training, including the issues listed in the above.  Each student should be given a personal copy of the written operator/guide.  The PI is also responsible for training users in the specific operation of individual units.

 

2.  Must ensure that all appropriate safety procedures are followed, that any safety devices (interlocks etc.) are functioning properly.

3.  Must supervise or otherwise ensure the adequate supervision of users, visitors and service personnel as appropriate, and provide adequate security to prevent unauthorized use.

4.  Must correct and control all equipment and physical/chemical hazards as appropriate.

 

5.  Must ensure that dosimetry is worn as appropriate.

 

6.  Must ensure that the unit is never repaired or the housing opened without first obtaining written permission from Campus Radiation Safety.  Radiation Safety must be contacted post repair to request a survey; the unit may not be put into service (used) until Radiation Safety has determined it to be safe.

 

7.  Must ensure that all personnel demonstrate competency in the use of the unit prior to allowing the personnel to use the unit without supervision.

 

RESPONSIBILITIES OF THE PERSONNEL/STUDENTS

 

1.  Must be authorized and trained appropriately to operate a potentially hazardous unit.

 

2.   Dosimetry must be worn if assigned by the Radiation Safety Office.

 

3.  Must adhere to all the above, and all appropriate rules and procedures.

 

4.  Must immediately report accidents, potentially dangerous situations or unusual occurrences or malfunctioning equipment to the supervisor and/or safety personnel.
